What is the Data Entry App?

Web-Based Entry

Accessed on a desktop or laptop browser — not the Android Capture app. Used to submit the monthly aggregate health dataset for each facility.

Monthly, One Per Facility

One form per facility per month. The dataset is Health facility data collection tools (Health facility level) — a custom HTML form with multiple sections.

15-Day Timeliness Window

Submissions within 15 days of the period end are marked "on time." After that, data can still be entered but is counted as late in reports.

Data Entry vs Capture App

📝 Data Entry App (Web)

  • Used on a computer browser
  • Submits the monthly aggregate dataset
  • One form per facility per month — totals and counts
  • Data shows in Standard Reports and Reporting Rate Summary
  • Needs: Data Entry role + org unit assignment
  • Example: Total ANC visits in April at Patarasi 01

📱 Capture App (Android)

  • Used on an Android phone
  • Submits individual event records (one per patient/visit)
  • 9 programmes — PPAs, FP Simulation, Mentorship, etc.
  • Data shows in Line Listing and Dashboards
  • Needs: Data Entry role + programme group + org unit
  • Example: One FP counselling event for a specific client
Rule: Aggregate monthly totals → Data Entry (web). Individual event records → Capture (Android). Both are needed — they serve different purposes.

Using the Data Entry App

Select your facility, open the monthly form, fill in the data, and complete

Step-by-Step — Submit Monthly Data

Open the Data Entry App

Apps grid ⋮⋮⋮Data Entry. The app opens with an org unit tree on the left and a selector panel at the top.

Select Organisation Unit

Expand the tree on the left: Nepal → Karnali Province → District → Municipality → click on the ward / health facility you are entering data for.

Select Dataset & Period

In the top selector: Dataset → Health facility data collection tools (Health facility level). Period → choose the correct month and year. The form loads automatically below.

Fill in the Form

Enter values in each field. Cells turn green when saved and yellow while saving. Scroll through all sections — the form has multiple parts.

Click "Complete"

When all sections are filled, click Complete at the bottom. A confirmation dialog appears — click Complete again. The Reporting Rate Summary will now show 100% for this facility and period.

Data Entry Tips & Notes

Auto-Save ≠ Complete

Each cell saves automatically as you type — but the form is not counted as submitted until you click Complete. A form with all cells filled but not Completed still shows 0% in Reporting Rate Summary.

Editing After Completing

Click Incomplete to reopen the form. Make your corrections — values auto-save. Click Complete again when done. This does not reset the Reporting Rate count.

No Expiry — Data Can Always Be Edited

This dataset has no expiry days set. Forms can be submitted or corrected at any time. However, anything submitted after 15 days of the period end is marked late.

Who Can Enter Data

Users need the Data Entry role and must be assigned to the org unit in their account. If the form or dataset doesn't appear, contact admin to check role and org unit assignment.

Common Issues & Fixes

Dataset not visible in dropdown

Your account is not assigned to this dataset. Ask admin to assign Health facility data collection tools to your user or user group.

🌳

Org unit not visible in the tree

Your account doesn't have the facility assigned as a capture org unit. Admin needs to add it under User management → Org units.

🔄

Form shows old / cached data

Hard-refresh the page (Ctrl+Shift+R / Cmd+Shift+R). If the problem persists, clear browser cache and reload.

⚠️

Cells not saving (stay yellow)

Check internet connection. If offline, values won't save — wait for reconnection. Do not close the browser tab until all cells turn green.

Reporting Rate still shows 0% after filling the form

You filled the fields but did not click Complete. Reopen the form and click Complete to register the submission.

Click Complete → confirm → done

Reports → Reporting Rate Summary → confirm 100%

Deadline: Submit within 15 days of the month end to be counted as on time. For April 2026, the on-time deadline is 15 May 2026.
