Monthly aggregate data submission for health facilities
Accessed on a desktop or laptop browser — not the Android Capture app. Used to submit the monthly aggregate health dataset for each facility.
One form per facility per month. The dataset is Health facility data collection tools (Health facility level) — a custom HTML form with multiple sections.
Submissions within 15 days of the period end are marked "on time." After that, data can still be entered but is counted as late in reports.
Select your facility, open the monthly form, fill in the data, and complete
Apps grid ⋮⋮⋮ → Data Entry. The app opens with an org unit tree on the left and a selector panel at the top.
Expand the tree on the left: Nepal → Karnali Province → District → Municipality → click on the ward / health facility you are entering data for.
In the top selector: Dataset → Health facility data collection tools (Health facility level). Period → choose the correct month and year. The form loads automatically below.
Enter values in each field. Cells turn green when saved and yellow while saving. Scroll through all sections — the form has multiple parts.
When all sections are filled, click Complete at the bottom. A confirmation dialog appears — click Complete again. The Reporting Rate Summary will now show 100% for this facility and period.
Auto-save, completing, editing, and access troubleshooting
Each cell saves automatically as you type — but the form is not counted as submitted until you click Complete. A form with all cells filled but not Completed still shows 0% in Reporting Rate Summary.
Click Incomplete to reopen the form. Make your corrections — values auto-save. Click Complete again when done. This does not reset the Reporting Rate count.
This dataset has no expiry days set. Forms can be submitted or corrected at any time. However, anything submitted after 15 days of the period end is marked late.
Users need the Data Entry role and must be assigned to the org unit in their account. If the form or dataset doesn't appear, contact admin to check role and org unit assignment.
Your account is not assigned to this dataset. Ask admin to assign Health facility data collection tools to your user or user group.
Your account doesn't have the facility assigned as a capture org unit. Admin needs to add it under User management → Org units.
Hard-refresh the page (Ctrl+Shift+R / Cmd+Shift+R). If the problem persists, clear browser cache and reload.
Check internet connection. If offline, values won't save — wait for reconnection. Do not close the browser tab until all cells turn green.
You filled the fields but did not click Complete. Reopen the form and click Complete to register the submission.
Apps grid → Data Entry
Expand org unit tree → click your ward / facility
Dataset dropdown + month/year → form loads
Enter all values — wait for green cells
Click Complete → confirm → done
Reports → Reporting Rate Summary → confirm 100%
Select facility → Select period → Fill form → Complete → Verify
